Summary

Home Care Nursing Professionals is a Home Nursing Agency that provides high quality tailored Home Nursing and Home Health Aide (HHA) solutions to individuals throughout the North Shore of Chicago. Specializing in providing advanced care for those with complex medical conditions, we have a team of high quality, experienced and qualified caregivers from HHAs to Registered Nurses (RNs) working for us. In providing our services our top priority is providing you or your loved ones with a high quality of life.

Our Story

Our Nurse Supervisor, Bogdan Tarau APRN, has over 15 years of experience caring for patients in both subacute home care settings and acute hospital based critical care settings. After caring for a ventilator dependent quadriplegic for over a decade, Bogdan decided he could do it better and together with John McCarthy, an experienced Home Nursing Agency Manager, formed Home Care Nursing Professionals. Now Home Care Nursing Professionals has been providing this individual with top quality nursing services for a number of years, helping him to stay as healthy and active as his condition allows, including helping him travel across the country under the direct supervision of our experienced team members.

Our Approach

As a Home Nursing Agency all of our services are provided under the direction of a medical professional. All of our caregivers are vetted, insured, bonded and trained. Our Nurses all hold either LPN, RN or higher designations and many of our HHAs are Certified Nursing Assistants (CNAs). Furthermore, all of our HHAs are trained in caring for those with Alzheimers. Our Nurse Supervisor is responsible for training and supervising all our nurses and HHAs. He utilizes a team-based approach in providing care through communicating with your complete care network including Family Members, Primary Care Physicians, Specialists, Dieticians, Social Workers, Physical and Speech Therapists to obtain relevant information in order to maximize your quality of care. Our team members will take pride in getting to know you and doing the best they can in providing safe and effective care as well as helping you meet your goals. As an agency we actively stay up to date on the best technologies and techniques that can help patients thrive in their homes, and periodically provide our staff with training in the areas we consider most relevant.

Home Health Aide Services

We don’t just provide nursing care, we also provide less complex caregiving services primarily through our HHAs. Our HHAs can care for children with special needs, individuals with Parkinson's disease and other movement disorders, Alzheimer's disease and other forms of Dementia, Multiple Sclerosis and other Neuromotor conditions, Chronic Obstructive Pulmonary Disease and other Chronic Conditions including Diabetes, Heart Disease, Pneumonia, Cancer and Arthritis. Our HHAs will primarily provide assistance with your Activities of Daily Living (ADLs) which among other things may include:

  • Ambulation and Transferring
  • Repositioning
  • Errands and Appointments
  • Meal Preparation
  • Feeding
  • Light Housekeeping
  • Transportation
  • Bathing, Grooming, Toileting
  • Dressing
  • Medication Reminding
  • Monitoring Vital Signs
  • Reporting and Documenting Changes in your Conditions

Our Process

Tailored Care Services

All of our services are performed in a dignified manner by a caregiver under the supervision of our Nurse Supervisor who will create and monitor a customized plan of care (POC) for your individual needs/goals. The extent of our caregiving services can range from hourly to around the clock, depending on your budget and needs. The level of supervision of our HHAs by our Nurses can vary based on your care needs, ranging from either all day instruction and supervision by an RN (as with our ventilator dependent quadriplegic patient) to primary care by an HHA with just a weekly check in with our Nurse Supervisor in the case of an individual with less complex medical requirements.

We seek to meet your physical needs through providing high quality caregiving services. But most of all we strive to allow you to have the highest quality of life possible, seeking to integrate your desires, needs, wishes and priorities into the care we provide. Going on a daily walk with you, assisting you in baking a pie with a friend or going to the grocery store to get the ingredients to make your favorite dinner are a few of the things we would enjoy helping you with.

You Contact Us

After your initial contact you can talk with our Nurse Supervisor either over the phone or through email about you or your loved one’s care needs and potentially set up a meeting.

Initial Consultation

Our Nurse Supervisor will visit you at your residence or treatment facility to provide a clinical assessment. Given your permission, he will consult with those in your care network so that we can understand your complete care needs and bring everyone on the same page regarding your care.

Your Input

After the initial assessment our Nurse Supervisor will meet with you again in order to set your care goals.

Plan of Care

Ultimately, we will use all of this information to develop your POC. This POC will not only take into account our Nurse Supervisor’s extensive nursing experience but also your input and those in your care network. The POC will focus on your current condition and work to optimize all factors including your diet, exercise and activity level. Once the POC is discussed with you and given your approval it will be signed off by your physician and implemented by a Caregiver trained by our Nurse Supervisor on your specific care requirements. The extent of supplemental nursing care required in lieu of or in addition to HHAs will depend on the complexity of your medical requirements.

Constant Oversight

Our Nurse Supervisor and caregivers will monitor you for changes in your care needs and desires. After input from you and your medical providers, our Nurse Supervisor will update your POC accordingly and implement any needed changes. Throughout the process of providing care, to the extent you require or desire, those in your care network will be apprised of the progress of your care. If at any time during your care process you have any concerns or issues you would like addressed you can contact our Nurse Supervisor directly and he will be happy to talk with you about your care needs.
